ADOPTION QUESTIONNAIRE

Hello and thank you for your interest in adopting a cat or kitten from Feral Fixers!

All of our cats and kittens are in foster homes. Adoptions are done from the foster home or our building in Lombard. Before we connect prospective adopters with the foster home to arrange a visit, we request that you complete the following questionnaire. Could you please provide answers to the questions below?

Before submitting your application, please make sure you have reviewed our single kitten policy on the main "Want to Adopt" page. We will not adopt out single kittens younger than 6 months unless you have another kitten or young resident cat in your home or if the adoptable kitten has been listed as needing to be adopted on its own in its posting.

Note that you will be unable to save information on this Questionnaire and then return to it at a later date, so please complete the form in one session.
